Transcript Request Form

If you need official transcripts use the form below. Transcript requests must be submitted in writing with a signature. Please click on the link below for a PDF of the actual form. Please include a check or money order when mailing in your request. If you have had an official name change since you attended ENC you must submit a copy of a legal document such as a driver's license validating that name change. Otherwise transcripts will be sent with original name. Writing LEGIBLY will be appreciated. Transcript questions should be directed to Yvonne Christensen (617-745-3879) . Cost for each transcript is $5. ($10 fee for same day requests).

Please note: Eastern Nazarene College and SCRIP-SAFE® International have partnered together to provide students and alumni with two new transcript services:

• Access to an online transcript ordering system through a secure website (Transcripts-on-Demand). Eastern Nazarene College charges $5.00 per transcript. Scrip-Safe charges a $3.00 processing fee for each recipient (transcript addressee.) Order updates will be emailed to the requestor and order status and history can be checked online. Any transcript request that will be paid by credit card must be submitted through Transcripts on Demand.

• Secure electronic transcript delivery. The electronic (secure PDF) format can be delivered to any email address including schools and universities, school districts, and prospective employers. PDF transcripts can be produced faster than paper transcripts and sending a PDF transcript eliminates mailing time. Requestors will receive notification of delivery by email.
